The Town of Queen Creek uses GovQA to manage public records. GovQA is a cloud-based software that helps government agencies manage public records and requests while helping agencies comply with state and local laws as well as federal HIPAA laws.

Unless explicitly communicated, all Fire, EMS and Medical Billing records will be completed using the GovQA portal. If you would like to receive your records in a different manner, please select the appropriate delivery method when making your request.

Records Release Authorization Requirements:

• EMS records are protected under state and federal law. If you are requesting a record for a deceased individual, please include a copy of the patient's death certificate or coroner's report when requesting records through the General Records Portal. 
• If requesting records under any other conditions, an Authorization to Release Medical Records signed by the patient, patient's guardian, Medical Power of Attorney, or Court Appointed Guardian with appointee papers is required.

There is no charge to review public documents or receive them digitally. However, fees may apply for photocopies, audio tapes, and other materials.
